Requisition form
Download, print, and send it with each specimen. Fax orders to (877) 369-4958.
Results
Results are posted to the portal and faxed to your office.
Critical values
Phoned to the ordering provider as soon as they're verified.
STAT testing at no extra charge
Monday to Friday, 9 AM to 2 PM. Results in 2 to 4 hours. Call (773) 986-3541 to request a STAT pickup or draw.
Who we work with
We work with physician practices, nursing homes, rehabilitation centers, home health agencies, and urgent care clinics across Chicago. Our phlebotomists can draw on site at your facility or at patients' homes.
